Getting your service dog registered with Registered Service Dogs.Com could not be easier. It is fast and affordable.
STEP ONE - Sign up with our secure payment gateway. You will be given a choice of online check, credit card, or even personally submitting your payment in the form of check or money order.
STEP TWO - Once you have completed our registration form you will be directed to our Downloadable Forms page. Registered Service Dogs.Com requires that you get two simple forms filled out. These are:
- A form to be signed by your doctor stating that you need a service dog for medical reasons.
- A form to be filled out by your veterinarian stating that your dog is up to date with all necessary shots. Also, in the best determination of the Veterinarian your dog is not vicious and is not a danger to the public.
By filling out these two forms our company (Registered Service Dogs) will be able to issue you a Service Dog Certificate. While this certificate is certainly not required by law it will become a valuable resource in helping insure businesses and orgainizations maintain compliance to the law.
STEP THREE - Send the completed form (with payment if applicable) to Registered Service Dogs.Com Your certification card will then be sent to you within 3 business days. |
